Discharge Co-ordination
The Case Management team at the NSIC are responsible for implementing a service which achieves a safe, high quality discharge for patients back into their community. The team contributes to the interdisciplinary planning required before patients are ready for discharge. It also leads and facilitates in the resolution of sometimes complex community issues through liaison with the patient’s local health provider and local Housing and Social Service Departments in order to formulate appropriate care plans to meet the needs of each patient.
The team case manage all newly injured patients and provide the necessary networks for patients to interact with specialist advisers for employment, housing benefit and care needs.
